Please read the following information before completing the request form. This should be done at least 3 weeks prior to your final oral exam. All of the required fields must be filled out.
- Committee Requirements: You are required to have at least 4 committee members, including your advisor. One committee member must be from outside the major field (i.e., outside of major path) If all committee members are from Chemistry, please indicate each faculty member’s path association in the notes field at the bottom of the form. In addition, you must indicate whether each faculty member is a reader or nonreader. At least 3 committee members must be designated as readers. For more information, see the Graduate School Academic Policies & Procedures (AP&P) website
- Grades: Please check your grades prior to submission to ensure that there are no NR, P, or I grades as the Graduate School will not approve the warrant with these grades. If you have a NR, P, or I, contact the appropriate professor to have the grade(s) changed.
- Changes: If anything changes with your defense, please email gradprogram@chem.wisc.edu.
If you are a student who transferred into the Chemistry PhD Program at UW, you will need to include information about your coursework from your previous graduate institution. The Graduate School will use this information to verify that you have met the Minimum Graduate Degree Credit and Coursework Requirements. You will need to enter information for up to five graduate courses that you completed while at your previous graduate institution. You will need to include the following information for each course:
- Institution
- Course Name
- Course Number
- Academic Term
- Credits
The Graduate Program staff will submit the request to the Graduate School and you will receive an email that the warrant has been submitted.
When the Graduate School approves the request, you will be notified that the request was approved and your final oral exam packet from the Department of Chemistry will be emailed to you.
Once you have passed your final defense, request signatures for your warrant through the Graduate Student Portal.
Announcing your PhD dissertation defense date to the department: after submitting the warrant request, please fill out the pdf titled Ph.D. dissertation defense form. One week before your defense, submit the Thesis Defense Information form. (Please note: the text submitted in this form will be sent as entered to the Department’s email list.)
